To bill care of patients who use the Medicare Hospice Benefit:

• For Care Not Related to Terminal Illness:
  Bill Medicare Part B; include GW modifier.
• For Care Related to Terminal Illness*:
  Bill Montgomery Hospice; include GV modifier. (Exception: Bill attending physician's professional services to Medicare Part B).

* Montgomery Hospice must pre-approve care. Physician must be the attending physician or a contracted consultant of Montgomery Hospice.

Billing Care of Medicaid Patients in Extended Care Facilities:

As soon as a Medicaid beneficiary residing in an extended care facility enters hospice, billing Medicaid for all aspects of that patient's care becomes the hospice's responsibility.

Montgomery Hospice passes through to the facility all of the Medicaid reimbursement that the hospice receives for room, board and other non-hospice services.
