Position Summary:

The Clinical Team Manager leads an interdisciplinary team of Nurses, Social Workers, and Chaplains in providing excellent end of life care that is patient and family centered.  The Clinical Team Manager has the responsibility and authority for planning, coordinating, implementing, and evaluating patient care services and staff education related to hospice care to provide for the safety and quality of care, treatment, and services.  Responsible for managing the day-to-day clinical operations of the assigned Hospice at Home interdisciplinary team.

Duties and Responsibilities:

  • Supervises members of the assigned interdisciplinary team, providing direct and daily supervision. This includes joint patient visits to provide oversight and support to meet organizational standards of care.
  • Responsible for assigning members of interdisciplinary team appropriately to meet patient needs and minimize travel time.
  • Proactively manages to prevent service failure with patient, family and facilities, ensuring immediate customer service recovery.
  • Responsible for timely annual evaluations and ongoing evaluations of all team members.
  • Monitors and conducts chart reviews on an ongoing basis. Reports, as requested, to QAPI.
  • Assists in the planning, implementation, and evaluation of in-service and continuing education programs.

Minimum Qualifications:

  • Graduation from an accredited nursing program. B.S.N preferred.
  • Current State of Maryland Nursing License, CHPN required.
  • Minimum of two to three years of hospice nursing experience, previous management experience required.
  • Good understanding of hospice and home care principles within the scope of Palliative Medicine.
  • Demonstrated knowledge of state, federal and regulatory standards as applied to hospice organizations.
  • Familiarity with interpersonal dynamics with the ability to maintain effectiveness in an emotionally stressful environment and demonstrated leadership.
