Clinical Nurse Educator / APPLY

Position Summary:
The Clinical Nurse Educator supports our clinicians through education, clinical mentoring, coaching and Quality-driven activities. In conjunction with the Hospice leadership team, the Clinical Educator will help to design, implement, and provide training to Hospice team members. Serves as a role model in advancing clinical practice to achieve better patient outcomes. Is responsible for the orientation and precepting of new field clinicians.
Duties and Responsibilities:
- Responsible for the orientation and precepting of new nurses.
- Develops precepting program and orients nurse preceptors.
- Serves as a clinical education mentor for all clinicians.
- Advocates for advancing clinical practice to achieve better patient outcomes.
- Researches best practice and promotes excellence in evidence-based practice and quality performance, integrating new knowledge into educational programs.
- Works with QAPI and Clinical Directors to ensure all education training and materials provided to staff are compliant with Federal, State, and local regulatory requirements
- Oversees the annual education curriculum for clinicians in RELIAS, our web-based training program, and designs the annual training plan for each clinical discipline.
- Serves as field educator, providing feedback and suggestions to implement best practices and standardize procedures.
- Conducts regular documentation review for new and current nurses to identify areas for improvement as needed.
- Serves as an on-site clinical resource to field staff with complicated patient cases. Makes time for time-sensitive consultation needs.
- As a member of the Staff Development Team, participates in the planning, implementation and evaluation of the education process of the organization.
- Assists with the development, implementation, and evaluation of New Employee Orientation.
- Educates all new employees on using the timekeeping and mileage system appropriately. Provides refreshers to current staff regularly.
- Ensures all relevant skills and competencies are introduced during orientation and validated during nurses’ precepting experiences.
Minimum Qualifications:
- Current state of Maryland RN license.
- Bachelor’s degree in nursing required. Master of Science in Nursing Education preferred.
- CHPN is required within one year of position acceptance.
- Five or more years of healthcare experience.
- Advanced knowledge of clinical expertise in hospice and palliative care.

Position Summary:

Position Summary:
Promotes the value of hospice services offered by Montgomery Hospice by maintaining a professional presence in a hospital setting to facilitate the referral of patients to Montgomery Hospice from physician practices, hospitals nursing homes, long-term care facilities, other community agencies and managed care companies. Provides information/education related to hospice and palliative care to patients, families, and hospital staff.
Duties and Responsibilities:
- Promotes an increased understanding of MH services and programs to appropriate health-related audiences through one-on-one contact, attends meetings and responds to inquiries.
- Meets with physicians, hospital discharge planners, nursing home social workers, and liaisons to assure services are meeting needs of referred patients and families.
- Attends discharge planning rounds, educational programs, case conferences and staff meetings at hospitals as appropriate.
- Maintains records of all contacts and requests for information.
- Participates in assessing, planning, implementation and evaluating policies to ensure efficient referral processes.
Minimum Qualifications:
- Must meet one of the following requirements: 1) Have an active Maryland or multi-state Registered Nurse license, 2) have an active Maryland Licensed Practical Nurse license, 3) have an active Maryland LMSW license, 4) have a bachelor’s degree in pastoral counseling or equivalent from an accredited professional theological school, seminary, or college, or 5) have a bachelor’s degree in business, marketing or related field.
- Three years of recent clinical experience required. Some hospice experience required. Knowledge of Montgomery County and its medical community is helpful.
- Knowledge of state, federal and regulatory standards as they apply to hospice organizations.
- Excellent written and communications strongly preferred.
- Access to and ability to drive a car for local travel appointments.
- Computer proficiency required.

Position Summary:
Responsible for the provision of a full range of spiritual care services to patients and families at Casey House/ and or Hospice at Home as assigned. Serves as a primary member of an interdisciplinary team, assisting other disciplines on the team in understanding spiritual factors related to the patient’s and the family’s comfort. Explains hospice services and benefit to referred patients and families; obtains consents for hospice care. Works in collaboration with other primary team members to coordinate care of the patient and family.
Duties and Responsibilities:
- Coordinates with the interdisciplinary team members to develop and implement a plan of care that includes potential problems and areas for spiritual care interventions.
- Evaluates emotional, social, spiritual, and religious factors to determine the capacity of the patient and caregivers to cope with terminal illness and death.
- Responsible for complete spiritual assessment delineating specific problems, goals, and interventions.
- Assist in the discharge planning from hospice services when care is no longer appropriate.
- Conducts special services for patients and families whenever possible and appropriate.
Minimum Qualifications:
- Minimum 3 years of experience in professional spiritual role, including work with patients and families dealing with life-limiting illness and death.
- Master’s degree in Divinity, Hebrew Letters, Pastoral Counseling or equivalent counseling experience required.
- 4 Units of CPE or equivalent counseling experience.
- Good standing in denomination and endorsed or eligible to be endorsed for spiritual care.
Social Worker/ APPLY

Position Summary:
Provides full range of social services to patients and families, including counseling in issues related to death and dying, coping, crisis intervention and community and economic resources. Explains hospice services and benefit to referred patients and families; obtains consents for hospice care. Works in collaboration with the other primary team members to coordinate care for the patient and family. Provides periodic on-call support as required.
Duties and Responsibilities:
- Evaluates the social, emotional, and environmental factors to determine the capacity of the patient and caregivers to cope with terminal illness and death.
- Participates in the completion of a psychosocial assessment delineating specific problems, goals, and interventions.
- Coordinates with interdisciplinary team members to develop and implement a plan of care that includes potential problems and areas in social work interventions.
- Provides initial assessment of hospice patients.
- Presents hospice services to potential hospice patients and to other healthcare professionals.
Minimum Qualifications:
- Masters of Social Work from an accredited graduate program.
- Licensed Social Worker with the state of Maryland. LCSW-C preferred. Certification as an APHSW-C encouraged after 2 years of hospice experience.
- Minimum of two years post-graduate clinical experience. Previous hospice experience preferred.
- Excellent listening and communication skills and effective interpersonal skills.
- Requires calmness and the exercise of sound judgment in emergency situations.

Administrative
Executive Assistant/ APPLY

Position Summary:

Duties and Responsibilities:
- Drafts correspondence and written documents for President/CEO and Senior Leadership Group members.
- Designs and creates flyers, brochures, forms, certificates, and other materials as requested.

Position Summary:
Works in accordance with the patient’s Plan of Care and supervised by the Registered Nurse, provides personal care for patients, and assists with their activities of daily living.
Duties and Responsibilities:
- Follows the hospice aide plan of care developed for each patient, under the supervision of the Hospice Nurse.
- Observes and reports any changes in patient’s condition immediately to Hospice Nurse.
- Completes documentation for each visit as directed.
- Continually assesses own skill and knowledgebase and actively seeks additional training through available resources.
Minimum Qualifications:
- Current certification from the State of Maryland as Nursing Assistant (AIDE) required; certification as Hospice and Palliative Nursing Assistant preferred.
- At least 2 years of practical experience in a hospital, nursing home or home care program. Home care or hospice experience preferred. Must have been providing similar services within the past two years.
- Communication skills: ability to read, write, and verbally report clinical information to patients, caregivers, and hospice staff. Requires ability to assimilate easily into emotionally charged environment. Demonstrates gentleness and empathy toward patients. Must be able to demonstrate proficiency in required skills.
- Will need to lift, position and transfer patients safely. Manual dexterity and mobility needed, particularly in emergency situations. Must be able to follow written and verbal directions. Job can be emotionally stressful.

Montgomery Hospice and Prince George’s Hospice offers generous benefits and ongoing support and development for the people who give so much of themselves to serve our community.
Our benefits include:
Comprehensive medical coverage for individuals and dependents is available for staff scheduled to work 30 hours or more per week. You may select from HMOs, Preferred Provider Organizations or Point of Service plans. Coverage begins the first day of the month following your date of hire.
Coverage is available for you and your dependents. Coverage includes preventive, basic and major services.
Employees who are scheduled for 30 hours or more are automatically covered by short-term and long-term disability coverage. The organization will pay the full premium for this valuable benefit.
Vision benefits are available to you and your family for employees regularly scheduled for 20 hours or more per week.
Employees scheduled to work 16 hours or more are eligible for prorated paid leave.
- Employees accrue paid vacation, at the rate of 13 days for the first year, increasing to 20 days in the fifth year.
- Employees accrue 10 days per year of sick leave to be used for yourself and family illnesses.
- We offer 7 paid holidays each year and 4 personal days.
- We offer up to 5 days of bereavement leave.
Montgomery Hospice and Prince George’s Hospice recognizes the importance of continuing staff development, and supports a program of education and in-service training for hospice staff. This includes tuition for conferences and workshops and paid education days.
