Position Summary:
Responsible for the provision of full range of spiritual care services to patients and families at Casey House/ and or Hospice at Home as assigned. Serves as a primary member of an interdisciplinary team, assisting other disciplines on the team in understanding spiritual factors related to the patient’s and the family’s comfort. Works collaboratively with other primary team members to coordinate care of the patient and family.

Position Summary:
The Consultant Dietitian shall provide the Facility with professional consultation in accordance with the State regulations, dietetic standards of practice, and their own professional judgement. Duties and Responsibilities:
- Acts as the medical resource for the interdisciplinary team.

Minimum Qualifications:

- Develops effective working relationships with representatives from other agencies who provide services for assigned patients.
- Provides nursing services to hospice patients by effectively solving problems using critical thinking skills and able to demonstrate proficiency in anticipating symptom management needs.
- Provides initial assessment of hospice patients by coordinating with other disciplines as appropriate.
- Completes all documentation within agency guidelines and in accordance with agency guidelines.
Minimum Qualifications:
- Graduation from an accredited nursing program. B.S.N preferred.
- Current State of Maryland Nursing License. CHPN preferred.
- Two years of recent clinical experience, preferably with critical care or previous experience in a hospice or homecare program.
- Requires strong critical thinking skills and EMR experience.
- Possess excellent verbal and written communication skills, and be able to communicate effectively with patients and families under stress.

Position Summary:
Assesses patients and families who request admission to Prince George’s Hospice. Provides a comprehensive assessment of patients to be admitted and initiates the plan of care consistent with standards of practice and functions in other nursing capacities as needed.
Duties and Responsibilities:
- Accurately assesses patients and family needs
- Develops an effective working relationship with representatives from other agencies who provide services for assigned patients.
- Identifies and recommends medical equipment and supplies that will assist in maintaining patient’s comfort and caregivers’ ability to provide care.
- Identifies the need for additional services, including nutritional assessment, physical therapy, occupational therapy and speech therapy and initiates appropriate referrals.
- Assesses need for and educates caregivers on safety and emergency preparedness, including safe use of medical equipment.
- Conveys physician’s requests to appropriate staff on appropriate forms.
Minimum Qualifications:
- Current state of Maryland Nursing License. CHPN preferred
- Graduation from an accredited nursing program. B.S.N preferred
- Two years of recent medical-surgical experience, preferably with oncology/multi-system failure.
- Previous hospice or home experience strongly preferred.

Comprehensive medical coverage for individuals and dependents is available for staff scheduled to work 30 hours or more per week. You may select from HMOs, Preferred Provider Organizations or Point of Service plans. Coverage begins the first day of the month following your date of hire.
Coverage is available for you and your dependents. Coverage includes preventive, basic and major services.
Employees who are scheduled for 30 hours or more are automatically covered by short-term and long-term disability coverage. The organization will pay the full premium for this valuable benefit.
