Nursing Services

Nurse Practitioner / APPLY

Position Summary:

Licensed Independent Provider who works as a member of the Medical Team to establish, review, coordinate and manage the medical care provided to the patients of Montgomery Hospice and Palliative Medicine Consultants. Assures that the quality of medical care is appraised and maintained.

Duties and Responsibilities:

  • Acts as the medical resource for the interdisciplinary team.
  • Makes in person home visits and telehealth visits to Hospice and PMC patients in Montgomery and Prince George’s County.
  • Consults with attending physicians regarding pain and symptom control.
  • Assures care provided is consistent with the plan of care and standard practice through the development and review of patients’ plans of care.
  • Provides consultative services to hospice staff and attending physicians including timely assistance to staff in providing appropriate care.

Minimum Qualifications:

  • Master’s Degree or higher, current Advanced Practice Board Certification and Maryland APRN license. Family Nurse Practitioner and Advanced Certified Hospice and Palliative Nurse certification preferred.
  • Three or more years of recent clinical experience as a Nurse Practitioner. Experience in Hospice and Palliative Care, Geriatric Medicine, Oncology, Acute Care or Home-Based Primary Care preferred.
  • Advanced clinical assessment and symptom management skills with knowledge of current palliative medicine standards and practice required.
  • Driver’s license and private means of transportation
  • Active NPI, CDS and DEA numbers required for prescribing. Enrollment in Medicare and Maryland Medicaid preferred. Eligibility to participate with Medicare, Medicaid and private insurers is required for billing.
  • Ability to learn and utilize 2-3 fully electronic medical records on a daily basis and to electronically prescribe medications.

Hospice RN / APPLY

Position Summary:

Functions as a member of an interdisciplinary team, responsible for providing primary nursing care to assigned patients. Responsible for coordinating the interdisciplinary team to ensure adequate assessment, planning, and implementation of the patient and family’s plan of care consistent with standards of practice.

Duties and Responsibilities:

  • Provides primary nursing services for assigned patients in the community.
  • Provides case management for hospice patients.
  • Assesses, plans, implements and evaluates nursing plans of care for assigned patients.
  • Completes all documentation using electronic medical records system.
  • Maintains contact with patient’s primary physicians.

Minimum Qualifications:

  • Graduation from an accredited nursing program. B.S.N preferred.
  • Current State of Maryland Nursing License.
  • Hospice certification preferred.
  • Two years of recent clinical experience, preferably with critical care or previous experience in a hospice or homecare program.

Hospice RN – Extended Hours / APPLY

Position Summary:

Responsible for providing patient care services. Makes planned and unplanned visits as assigned. Responsible for implementing plan of care consistent with standards of practice. May assess patients and families who request admission to Montgomery Hospice to determine appropriateness for admission to hospice.

Duties and Responsibilities:

  • Make planned and unplanned visits to current hospice patients.
  • Ensures implementation of the plan of care.
  • Develops effective working relationships with representatives from other agencies who provide services for assigned patients.
  • Provides nursing services to hospice patients by effectively solving problems using critical thinking skills and able to demonstrate proficiency in anticipating symptom management needs.
  • Provides initial assessment of hospice patients by coordinating with other disciplines as appropriate.
  • Completes all documentation within agency guidelines and in accordance with agency guidelines.

Minimum Qualifications:

  • Graduation from an accredited nursing program. B.S.N preferred.

Position Summary:

Promotes the value of hospice services offered by Montgomery Hospice by maintaining a professional presence in a hospital setting to facilitate the referral of patients to Montgomery Hospice from physician practices, hospitals, nursing homes, long-term care facilities, other community agencies and managed care companies. Provides information/ education related to hospice and palliative care to patients, families and hospital staff.

Duties and Responsibilities:

  • Promotes an increased understanding of MH services and programs to appropriate health-related audiences through one-on-one contact, attends meetings and responds to inquiries.
  • Meets with physicians, hospital discharge planners, nursing home social workers, and liaisons to assure services are meeting needs of referred patients and families.
  • Attends discharge planning rounds, educational programs, case conferences and staff meetings at hospitals as appropriate.
  • Maintains records of all contacts and requests for information.
  • Participates in assessing, planning, implementation and evaluating policies to ensure efficient referral processes.

Minimum Qualifications:

  • Graduation from an accredited Nursing program. B.S.N preferred or M.S.W with LCSW-C Maryland License.
  • Minimum 3 years of recent clinical experience required. Some Hospice experience required.
  • Knowledge of state, federal and regulatory standards, including Joint Commission standards as it applies to hospice organizations.
  • Excellent written and communications strongly preferred.

Counseling Services 

Bereavement Counselor / APPLY

Position Summary:

Provides a range of grief counseling services to hospice family members and community, including phone and online counseling support (and in-person when it becomes safe and appropriate).  Serves as a member of the interdisciplinary team to provide team support, assisting with grief-related transitions after the patient’s death.  Provides professional co-facilitation of grief groups and workshops throughout the year (including some evenings).  Works closely with a team of counseling colleagues to coordinate care; provides accurate and appropriate documentation for all counseling and support.

Duties and Responsibilities:

  • Assesses the impact of loss in working with clients and works toward meaningful integration and healthy coping over time.
  • Manages a counseling caseload that includes scheduling assignments and providing complete documentation of specific issues and interventions.
  • Collaborates with grief counseling colleagues to provide safe, effective support groups and workshops through the year.
  • Provides a supportive time in interdisciplinary meetings for colleagues.
  • Works closely with grief counseling colleagues around coordination of care and ongoing education.

Minimum Qualifications:

  • Master’s Degree in Counseling or a counseling-related field
  • Previous hospice and/or grief counseling experience (including groups) preferred
  • Excellent listening and interpersonal skills in phone and video counseling
  • A basic understanding of loss and grief with some personal integration
  • Ability to work from home and have some facility with online communication platforms (Zoom, etc.)

Casey House 

Hospice RN/ APPLY

Position Summary:

Responsible for providing skilled nursing care to the terminally ill patients at Casey House. Responsibilities include assessing the needs of the patient and providing and documenting patient care in a caring and compassionate manner. To provide evidenced based nursing care to provide symptom management and emotional support in an inpatient setting. Exhibits expert end-of-life care and provides supervision to Hospice Aides.

Duties and Responsibilities:

  • Provides nursing care for hospice patients admitted to Casey House.
  • Implements nursing plans of care for assigned patients.
  • Completes all documentation within agency guidelines and in accordance with agency standards.
  • Required to work one summer and one winter holiday.

Minimum Qualifications:

  • Graduation from an accredited nursing program. B.S.N preferred.
  • Current State of Maryland Nursing License. CHPN preferred.
  • One year of recent clinical nursing experience in medical-surgical, oncology or critical care environment. Previous hospice experience preferred.
  • Excellent physical assessment, pain assessment and IV skills.
  • Requires strong critical thinking skills and EMR experience.
  • Possess excellent verbal and written communication skills, and be able to communicate effectively with patients and families under stress.

Our benefits include:

Comprehensive medical coverage for individuals and dependents is available for staff scheduled to work 30 hours or more per week. You may select from HMOs, Preferred Provider Organizations or Point of Service plans. Coverage begins the first day of the month following your date of hire.

Coverage is available for you and your dependents. Coverage includes preventive, basic and major services.

Employees who are scheduled for 30 hours or more are automatically covered by short-term and long-term disability coverage. The organization will pay the full premium for this valuable benefit.

Vision benefits are available to you and your family for employees regularly scheduled for 20 hours or more per week.

Life insurance is available for employees who are scheduled for 20 hours or more per week. You may cover yourself and your spouse from $10,000 to $300,000 depending on medical eligibility.

Employees scheduled to work 16 hours or more are eligible for prorated paid leave.

  • Employees accrue paid vacation, at the rate of 13 days for the first year, increasing to 20 days in the fifth year.
  • Employees accrue 10 days per year of sick leave to be used for yourself and family illnesses.
  • We offer 7 paid holidays each year and 4 personal days.
  • We offer up to 5 days of bereavement leave.
Confidential counseling and referral service is available to all employees, their spouses and dependents. Services are provided through LifeWork Strategies, a division of Adventist Healthcare and include 24/7 access to a toll-free help line, assessment by professional counselors, referrals to EAP counselors and community based resources. It also includes resource and referral services for child care, senior care, legal and financial consultation.
The organization has a policy for donating or requesting leave for employees who exhausted their own leave because of a health crisis of their own or a family member.
All employees are eligible to participate in a voluntary Tax Sheltered Annuity [403(b)] through Vanguard as a means of saving for retirement.

Staff will be reimbursed for hospice-related travel at the current IRS rate.
The agency encourages nurses to apply for certification as a hospice and palliative nurse (CHPN) and nursing assistants as hospice and palliative care nursing assistants (CHPNA) through the certification process of the National Board for Certification of Hospice and Palliative Nurses. Social workers are also eligible for a national Hospice and Palliative Certification. The organization will pay for the exam and award a 5% pay increase for certification.

Montgomery Hospice and Prince George’s Hospice recognizes the importance of continuing staff development, and supports a program of education and in-service training for hospice staff. This includes tuition for conferences and workshops and paid education days.
