Position Summary

The Intake Coordinator serves as the first point of contact for individuals inquiring about hospice services. This role is responsible for gathering referral information, coordinating with the admissions team, and ensuring a smooth and efficient admission process. The Intake Coordinator plays a vital role in communicating with patients, families, healthcare providers, and internal staff to facilitate timely access to compassionate end-of-life care.

Duties and Responsibilities

  • Receive and process incoming hospice referrals from hospitals, physicians, facilities, and families.
  • Demonstrate the ability to explain Montgomery Hospice services and programs, hospice philosophy, and eligibility criteria to all who call requesting information.
  • Obtain and verify all necessary demographic, clinical, and insurance information to determine hospice eligibility.
  • Enter and update patient information accurately in the Electronic Medical Record (EMR) system.
  • Record and track referrals through admission or termination of the referral.
  • Communicate with referral sources to obtain medical records and clinical documentation.
  • Provide information about payment provisions for Medicare, Medicaid, and commercial insurance during the admission process.
  • Work cooperatively with the Insurance Coordinator when verification of insurance coverage is needed.
  • Collaborate with the admissions team to schedule admissions.
  • Serve as a liaison between referral sources and the hospice admissions and clinical teams.
  • Develop effective working relationships with representatives from other agencies who provide services for assigned patients.
  • Maintain accurate logs, reports, and documentation in accordance with organizational policies and regulatory requirements.
  • Ensure compliance with all HIPAA, Medicare, Medicaid, and other regulatory guidelines.

Minimum Qualifications

  • High School Diploma or GED required. Associate’s or Bachelor’s degree in a healthcare-related field preferred.
  • One to three years of experience in healthcare intake, admissions, or customer service; hospice or home health experience preferred.
  • Familiarity with medical terminology and EMR systems.
  • Excellent verbal and written communication skills, including the ability to communicate effectively with individuals under stress.
  • Excellent organizational and computer skills required.
  • Strong attention to detail and data accuracy.
  • Ability to multitask and work in a fast-paced environment.
  • Demonstrates a compassionate, patient-focused approach to care.
