Position Summary:
The Professional Liaison is responsible for generating qualified hospice referrals to meet or exceed quarterly marketing and admissions goals. This field-based role involves frequent off-site engagement with physicians, hospitals, skilled nursing facilities, assisted living communities, and other potential referral sources. Through scheduled visits, in-services, and presentations, the Liaison educates healthcare professionals and community partners about hospice services to drive referrals and support organizational growth. The Liaison works collaboratively with clinical teams to ensure smooth and timely transitions for referred patients, serving as a key point of contact between the referral source, the patient, and the hospice team. They are responsible for accurately tracking referral activity and maintaining up-to-date records in the customer relationship management (CRM) system. Additionally, the Liaison conducts ongoing market analysis to identify referral trends, competitive activity, and growth opportunities. Participation in community events, health fairs, and outreach initiatives is also expected to raise awareness and visibility of hospice services.
Duties and Responsibilities:
- Drives qualified hospice referrals through direct, in-person engagement with physicians and care providers to meet or exceed quarterly sales targets and contributes to organizational revenue growth.
- *Spends a minimum of 75% of time off-site engaging with patients, families, and referral sources to educate them on Montgomery Hospice’s programs, services, and philosophy of care.
- *Meets regularly with physicians to solicit and secure hospice service orders and strengthen referral relationships.*d)Keeps current knowledge of hospice benefits covered by Medicare and other insurers.
- *Provides input for written material such as newsletters and brochures.
- Coordinates referrals to both Hospice at Home and Casey House.
- *Assists with the administration of requests through the Speaker’s Bureau.
- Meets with physicians, hospital discharge planners, nursing home social workers, and liaisons to ensure services meet the needs of referred patients and families.
- *Attends discharge planning rounds, educational programs, case conferences and staff meetings at hospitals as appropriate.
- *Addresses concerns and complaints quickly.
- *Maintains records of all contacts and requests for information.
- *Demonstrates knowledge of regulatory requirements and ensures compliance with all requirements.
- *Participates in assessing, planning, implementation, and evaluating policies to ensure efficient referral processes.
Minimum Qualifications:
- Bachelor’s degree in related field (preferred).
- Licensure/Certification: Must possess a valid driver’s license and have a clean driving record.
- Minimum 2 years of experience in healthcare sales or hospice/ home health marketing.
- Knowledge of Medicare, hospice benefit and end-of-life care strongly preferred.
- Excellent interpersonal, communication, and presentation skills.
- Self-motivated with strong organizational and time management abilities.
- Valid driver’s license and reliable, insured transportation required.
- Must have knowledge of state, federal and regulatory standards, including accrediting body standards, as applied to hospice organizations.
- Must have effective interpersonal skills, excellent written and oral communication skills.
- Access to and ability to drive a car for local travel to appointments.
- Computer proficiency required.
