If you are interested in becoming a Montgomery Hospice and Prince George’s Hospice volunteer, call the Volunteer Services Department at 301-921-4400. Our volunteer managers can share more about hospice volunteering and upcoming training dates. The forms needed for applying to volunteer are listed below.
Volunteers must be at least 18 years old and have access to reliable transportation.
Note that the trainings do fill up. It is important that you call first before filling out all the forms below.
Prospective adult volunteers will be asked to provide:
- Volunteer Application: Biographical Information & Statement of Interest – This form can be completed and submitted online below, or may be printed, completed, and returned to us by mail.
- Two Non-Family References: References should be asked to complete and mail the forms directly to Montgomery Hospice and Prince George’s Hospice’s Rockville office.
- Physician’s Statement: Only required for applicants who would like to volunteer in patient care; this form should be printed, completed, and returned to us by mail.
Prospective volunteers will also do an in-person interview at our offices with someone from our Volunteer Services staff.
